Assistant Director of Case Management

Job Summary:

The Assistant Director of Case Management is responsible for managing the case management staff, including hiring, terminating, supervision, scheduling, and orientation of new staff. They assist the Director of Case Management with day-to-day operations, attend meetings, and provide administrative coverage. The ideal candidate will have a Bachelor's degree in Nursing, a minimum of 5 years of post-licensure acute care RN experience, and a minimum of one year of nursing leadership experience.

Key Responsibilities:

  • Manage case management staff, including hiring, terminating, supervision, scheduling, and orientation of new staff
  • Assist the Director of Case Management with day-to-day operations
  • Attend meetings, including daily operations huddle, long length of stay at both campuses, escalation, and post-acute partnership meeting as needed
  • Co-facilitate case management staff meetings and provide administrative coverage for the Director of Case Management
  • Develop and implement strategic and operational case management initiatives, customer and staff satisfaction, quality management, certification, and accreditation activities, and professional practice activities
  • Oversight of SA system as it relates to case management

Requirements:

  • Bachelor's degree in Nursing
  • NYS Licensed/Registered (or eligible) RN
  • Minimum of 5 years post-licensure acute care RN experience
  • Minimum of one year nursing leadership experience
  • Prior Case Management experience and demonstrated competency in teaching/learning process, organizational skills, and computer literacy
  • Excellent written/oral communication, leadership, organizational, interpersonal, and analytic skills
  • CPR-BLS certification required by time of appointment

Preferred Qualifications:

  • National Certification in Case Management, Care Coordination, or Transitional Care
  • Masters degree in Nursing

Work Schedule:

Monday-Friday: Days with some weekend responsibilities
