Helio Health is seeking a skilled Case Manager to support its mission of promoting recovery from substance use and mental health disorders. This role is crucial in managing and coordinating services for patients/residents participating in or who have completed our treatment programs. The successful candidate will work in a Full-Time capacity at our HUD Housing program, Monday-Friday, 8:00am-4:30pm.
Key Responsibilities
- Screen and intake patients, providing orientation and assessment.
- Develop and implement treatment plans, including referrals and crisis intervention.
- Provide patient education, case management, and reporting.
- Collaborate with other professionals and community providers to ensure comprehensive care.
- Conduct home visits and train staff, volunteers, and students/interns.
- Participate in Quality Assurance activities, evaluating programs and identifying strengths/weaknesses.
Requirements
- High School Diploma/GED required, associate's degree preferred.
- Two years of experience in behavioral health and/or substance abuse preferred.
- Candidates with lived experience encouraged to apply.
- Valid NYS Driver's License required.
