Position: Speech Language Pathologist

CommuniCare Rehab, part of the CommuniCare Family of Companies, is seeking a dedicated Speech Language Pathologist (SLP) for a PRN role. This position offers the chance to work within our Chardon Healthcare Center, where our therapists are integral members of the team.

Qualifications:
  • A Bachelor's and/or Master's Degree in Speech-Language Pathology from an Accredited Institution.
  • A valid or pending State Speech Language Pathology License.

Key Responsibilities:

Your duties will include, but are not limited to:

  • Assessing and reviewing patients' medical records and evaluations as per physician directives, and formulating a tailored treatment plan based on individual needs.
  • Designing and implementing rehabilitative programs aimed at enhancing communication, swallowing, and cognitive abilities, along with administering and interpreting specialized speech assessments.
  • Educating residents, families, and caregivers on speech monitoring techniques and providing recommendations for safe swallowing practices.
  • Participating in interdisciplinary meetings to share clinical insights and offer recommendations to the rehabilitation team, as well as engaging in family education sessions.
