Job SummaryWe are seeking a highly motivated and compassionate Case Manager to join our team. As a key member of our Rapid Re-Housing program, you will provide housing-first and client-driven case management services to formerly homeless individuals. Your primary objective will be to assist clients in achieving stability in housing by connecting them with essential resources and services.
Key Responsibilities- Provide outreach services to program participants in their homes, emergency shelters, and other locations.
- Assist clients in identifying barriers and connect them with appropriate community resources.
- Work with a caseload to establish goals, assist clients in obtaining employment, and facilitate access to social services.
- Collaborate with crisis clinicians and other departments to find ways to support individuals with mental and behavioral health barriers.
- Keep accurate records, case management notes, and statistical data as required.
- Experience working or volunteering with diverse and vulnerable populations.
- Lived, work, or volunteer experience preferred, or a degree in a related field.
- Ability to lift 25 pounds, stand and walk for at least an hour at a time, and drive a vehicle.
- Ability to pass a pre-employment background check and drug screen.
