Monument Health Rapid City Hospital is seeking a skilled Registered Nurse - Case Management to join our team. As a case manager, you will play a critical role in ensuring the best possible outcomes for our patients.

About the Role

This is a travel RN case management position that offers the opportunity to work with a dynamic team of healthcare professionals. As a case manager, you will be responsible for coordinating patient care, developing and implementing care plans, and collaborating with other healthcare professionals to ensure seamless patient transitions.

Responsibilities
  • Coordinate patient care and develop individualized care plans
  • Collaborate with multidisciplinary teams to ensure comprehensive patient care
  • Conduct thorough assessments and identify patient needs
  • Develop and implement effective care plans to achieve optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare teams

Requirements
