Job Description

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as an LPN Community Health Specialist. In this role, you will be responsible for providing patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities

  • ">
  • Provide vocational nursing services under the supervision of an RN, to patients in their homes.">
  • Perform patient data collection in a holistic manner.">
  • Provide accurate and complete documentation.">
  • Schedule daily visits to facilitate patient needs.">
  • Plan patient care in consultation with Registered nurse.">
  • Assist with patient/caregiver teaching.">
  • Demonstrate flexibility in response to unexpected changes and the ability to adjust priorities and use resources.">
  • Maintain open communication with RN Case Coordinator and RN Supervisor.">
  • Demonstrate suitable time management skills.">
  • Meet agency's expectations regarding productivity.">
  • Participate in monthly case conferences.">
  • Participate in agency Performance Improvement activities.">
  • Demonstrate good time management skills.">
  • Be on call, as requested, RN as back up.">
  • Provide orientation to newly hired LPNs and host student LPNs.">

Qualifications

  • ">
  • Licensed Practical Nurse or Licensed Vocational Nurse.">
  • BLS Certificate.">
  • Valid Driver's License and Auto Insurance.">
  • One year of medical/surgical experience.">
  • Preferred qualifications: one year experience in community health nursing and thirty (30) hours of Florida Board approved IV Therapy Course.">

Compensation

">

The estimated salary for this position is $75,000 - $95,000 per year, depending on experience. Our comprehensive benefits package includes medical, dental, and vision insurance, 401(k) retirement plan, paid time off, and opportunities for professional growth and development.
