We are seeking a highly skilled and compassionate nurse to join our team. Your primary responsibility will be to clinically assess patients and facilitate their admission to our hospice care program. You will also work closely with healthcare partners, facilities, patients, and their loved ones to coordinate discharge planning and ensure seamless care transitions.

Responsibilities
  1. Conduct thorough clinical assessments of patients to determine eligibility for hospice care.
  2. Facilitate patient admissions and coordinate discharge planning with healthcare partners and facilities.
  3. Work closely with patients and their loved ones to address their needs and concerns.
  4. Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
  5. Maintain accurate records and documentation of patient interactions and care provided.
Requirements
  • Bachelor's degree in Nursing (BSN) or equivalent required; Master's degree preferred.
  • Minimum three years' experience in home care or hospice nursing required.
  • Previous experience in managed care, infusion care, and case management a plus.
  • Current RN licensure in North Carolina required.
  • Excellent communication and interpersonal skills necessary for effective collaboration and patient education.
Salary and Benefits

The salary range for this position is $85,000 - $105,000 per annum, depending on qualifications and experience.
