Job Summary:

Hospice Liaison Nurses at Novant Health play a critical role in assessing and admitting patients to our comprehensive hospice care program. These skilled professionals also facilitate discharge planning from acute inpatient settings, coordinating with healthcare partners, facilities, patients, and their loved ones. Our team is dedicated to delivering exceptional patient experiences, fostering teamwork, and promoting community involvement.

Responsibilities:
  • Deliver remarkable patient experiences in every dimension, every time
  • Foster a safe patient environment driven by the principles of 'First Do No Harm'
Qualifications:

To be successful in this role, you will need:

  • A High School Diploma or GED, with a Bachelor's Degree preferred
  • Minimum three years' experience in home care or hospice
  • Current RN licensure in the appropriate state
  • Excellent nursing assessment skills and judgment, as well as knowledge of Hospice philosophy
Estimated Salary:

$65,000 - $85,000 per year, depending on location and experience
