Job Overview: As a Home Care Case Manager RN, you will play a vital role in providing exceptional patient care and coordination services. This position requires a strong clinical background, effective communication skills, and a commitment to delivering high-quality care. Our team works collaboratively to ensure seamless transitions between care settings and provides comprehensive support to patients and families.

Key Responsibilities:
  • Assess patients' needs and develop individualized care plans.
  • Collaborate with healthcare teams to coordinate care and facilitate smooth transitions.
  • Provide education and support to patients and families to promote self-management and independence.
Requirements:
  • A Bachelor's Degree in Nursing is preferred; a High School Diploma or GED is required.
  • At least three years of experience in home care or hospice is necessary. Previous experience in case management is preferred.
  • A current RN licensure in the appropriate state is required.
  • Additional skills include excellent communication and interpersonal skills, knowledge of healthcare regulations, and proficiency in electronic medical records.
Salary Range: $90,000 - $105,000 per year
