Clinical Assessment Advisor

We are seeking a skilled Clinical Assessment Advisor to join our team at CenterWell Home Health, specializing in personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization.
As a key member of our care team, you will be responsible for ensuring that patients receive the highest level of care and support in their homes. Your primary focus will be on conducting initial and ongoing evaluations of patient needs, developing and implementing individualized care plans, and coordinating with healthcare professionals to ensure seamless care transitions.

Key Responsibilities- Conduct thorough assessments of patient needs, identifying areas for improvement and developing targeted interventions
- Develop and implement individualized care plans, incorporating input from patients, family members, and healthcare professionals
- Coordinate with healthcare teams to ensure timely and effective care transitions, including referrals to specialists and other community resources
- Maintain accurate and detailed records of patient interactions, progress, and outcomes
- Collaborate with colleagues to share best practices, address challenges, and improve overall care delivery
- Licensed Registered Nurse (RN) with current certification and active licensure
- Minimum 2 years' experience in clinical home care, preferably in a management or leadership role
- Excellent communication, interpersonal, and problem-solving skills
- Able to work effectively in a fast-paced environment with multiple priorities and deadlines
- Demonstrated commitment to quality care, patient safety, and customer satisfaction
CenterWell Home Health offers a competitive compensation package, including:
- $84,000 - $111,600 per year, based on experience and qualifications
