About the Role

We are seeking a skilled Clinical Care Coordinator to join our team at Eventus WholeHealth. In this critical role, you will serve as a liaison between our interdisciplinary care team and facility staff, ensuring smooth communication and collaboration to deliver exceptional care.

As a Clinical Care Coordinator, you will be responsible for coordinating resident assessments, developing treatment plans, and facilitating care transitions. Your expertise in population-specific interventions and psychotherapy techniques will enable you to provide personalized support to residents and families, promoting optimal outcomes and quality of life.

You will work closely with our multidisciplinary team, including physicians, nurse practitioners, and support staff, to ensure that all aspects of care are addressed. Your ability to communicate effectively and navigate complex care systems will be essential in this role.

About the Job

  • Coordinate resident assessments and develop individualized treatment plans.
  • Facilitate care transitions and communicate with facility staff.
  • Provide education and support to residents and families.
  • Document treatment plans and progress updates.
  • Participate in regular team meetings to discuss patient care and share best practices.

Requirements

  • Degree in social work or a related field (e.g., LCSW, LPCC, LCMHC, LMFT).
  • Certification in population-specific interventions (e.g., gerontology).
  • Experience working in long-term care settings.
  • Excellent communication and interpersonal skills.
  • Able to work collaboratively as part of a team.
