Job Title: Community Health Outreach Specialist

Upward Health is a leading provider of in-home, multidisciplinary medical care, offering 24/7 whole-person care to patients in need. As a Community Health Outreach Specialist, you will play a critical role in connecting patients with our services and helping them achieve their health goals.

Key Responsibilities:
  • Conduct direct outreach to patients via phone, in-person meetings, and other strategies to educate them about our services and support their health needs.
  • Enroll patients into our program and collect key data to ensure a seamless experience.
  • Manage a personal caseload of patients and develop effective strategies to engage them in their care.
  • Participate in weekly team meetings to stay up-to-date on best practices and share knowledge with colleagues.
  • Accurately document patient interactions and outcomes in our software system.
Requirements:
  • Excellent verbal communication skills and ability to build trust with patients.
  • Persuasive and confident in communicating our service offering and value.
  • Active listening skills and genuine compassion for others.
  • Quick thinking and ability to respond to questions and objections.
  • Organized and self-motivated with a strong work ethic.
  • Ability to work independently and meet established goals.
  • Comfortable using computer for documentation, communication, and organizing work.
  • Reliable transportation and ability to work flexible hours, including occasional night/weekend work.
  • Minimum of 3 years of work experience in sales, healthcare, or outreach roles.
