The Patient Engagement Coordinator (PEC) plays a vital role at Upward Health, acting as the main liaison for patients. The PEC is tasked with informing patients about our offerings and guiding them as they embark on their health journeys. This position demands strong communication skills and the ability to connect with patients through diverse outreach methods.

PRIMARY DUTIES:

  • Conduct outreach initiatives to patients through phone calls, face-to-face interactions, and other communication channels
  • Assist in enrolling patients into Upward Health's programs and gather necessary information
  • Oversee a caseload of patients while ensuring effective communication practices
  • Engage with enrolled patients to foster ongoing participation
  • Contribute to team meetings for continuous learning and development
  • Maintain accurate records in the company's digital systems
