About the Role

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our team as a Home Health Visits Specialist. As a Physical Therapist with BAYADA, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients in the comfort of their own homes.

Responsibilities
  • Conduct home visits to clients in designated geographic territories to provide physical therapy services.
  • Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
  • Develop and implement individualized care plans to promote clients' health, safety, and independent living.
  • Educate and instruct clients and their families on rehabilitative care and activities necessary for optimal recovery.
  • Accurately document observations, interventions, and evaluations to ensure seamless care management.
Qualifications
  • A current PA Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
