We are seeking a highly skilled and compassionate Physical Therapist to join our team at Pinnacle Home Health and Therapy Services. As a Home Health Physical Therapist, you will have the opportunity to work with adult and geriatric patients in their homes, providing personalized care and rehabilitation services.

About the Position

This is a full-time position that offers a flexible schedule, allowing you to work directly with your patients and schedule your visits around your own life appointments and events. You will have the autonomy to decide when you start and end your day, making it an ideal opportunity for those who value flexibility.

Responsibilities
  • Provide physical therapy services to adult and geriatric patients in their homes
  • Develop and implement personalized treatment plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify areas of improvement
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care
  • Maintain accurate and detailed records of patient progress and treatment plans
Requirements
  • Master's or Doctorate degree in Physical Therapy
  • Current licensure to practice as a Physical Therapist in the state
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Flexibility and adaptability in a fast-paced environment
