Key Responsibilities
Per Diem Position
Primary Duties:
- Conduct discharge planning assessments as required, ensuring timely execution.
- Build collaborative relationships with medical professionals and healthcare team members to enhance patient care and minimize length of stay.
- Engage in discussions with physicians regarding discharge planning requirements.
- Participate in bedside rounds with physicians whenever feasible.
- Work alongside healthcare team members to develop a cohesive and thorough discharge plan for patients.
- Create discharge plans that address patient and family concerns, healthcare status, available resources, age-specific issues, and diagnosis-related factors.
- Establish discharge plans based on assessments, including dual plans if necessary, and finalize arrangements prior to the conclusion of acute care.
- Provide appropriate referrals to various resources for patients and families.
- Conduct third-party payer reviews as needed within the specified timeframe and document findings.
- Must possess a Master's degree in social work from an accredited institution.
- Two to three years of experience in a healthcare-related field (hospital, agency, etc.) or prior discharge planning and/or community health experience is preferred. Candidates should be highly motivated, organized, adaptable, and detail-oriented with excellent communication skills.
- A valid social work license in New York State is required; screening certification is preferred.
- LMSW certification from the State of New York is mandatory; LCSW certification is preferred.
USD $53.75 per hour.
