Care Management Specialist

As a Care Management Specialist RN, you will play a vital role in ensuring the seamless transition of patients from hospital to home. Your primary responsibility will be to develop and implement individualized care plans that address the unique needs of each patient.

Key Responsibilities:
  • Conduct thorough assessments of patients to identify potential barriers to care
  • Develop and maintain accurate and up-to-date patient records
  • Collaborate with healthcare teams to ensure timely and effective discharge planning
  • Provide education and support to patients and their families to promote healthy behaviors and self-management

Requirements:
