- Communicate Effectively: Provide clear and concise information to patients and their representatives, and capture signatures as necessary.
- Document Patient Interactions: Maintain accurate and detailed records of patient interactions, including follow-up IMM and Choice Letters.
- Collaborate with Care Team: Maintain regular communication with Social Workers, Care Managers, and Care Management Leadership to ensure seamless patient care.
- Provide Excellent Customer Service: Demonstrate a commitment to customer service standards, maintaining positive interpersonal relationships and a professional image in all interactions.
- Education: High School Diploma or GED
- Experience: 2 years of healthcare experience preferred, with knowledge of care management functions
