Affirmed Home Care is seeking a skilled Licensed Practical Nurse LPN to join our team in Fairfield County Connecticut.

Responsibilities may include:
- Providing Best Practice nursing care to patients in their homes
- Checking vital signs and documenting in a complete and confidential manner
- Evaluating patient wellbeing and reporting needs to the Director of Clinical Nursing or emergency services
- Providing personal care and meals
- Performing advanced nursing tasks such as chest tubes IVs colostomy urinary catheter management
- Providing emotional support to clients and families in times of crisis
Requirements include a valid LPN license annual physical within one year PPD or Quantiferon within one year or chest x ray within five years and COVID vaccine card.
