Licensed Practical Nurse

At Affirmed Home Care, we are seeking a qualified Licensed Practical Nurse to join our team. As an LPN, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being.
Responsibilities:
Travel to patients' homes and provide nursing care
Check vital signs and document patient information
Evaluate patient wellbeing and report any needs to the Director of Clinical Nursing or emergency services
Provide personal care and meals
Administer medications and perform other nursing tasks as needed
Requirements:
Valid LPN license
Annual physical within one year
PPD or Quantiferon within one year or chest x-ray within five years
COVID vaccine card
