Behavioral Health Care Advocate

Job Summary:
We are seeking a highly skilled Behavioral Health Care Advocate to join our team. The successful candidate will be responsible for facilitating and providing linkage to behavioral health services in the system of care to justice-involved clients.
Key Responsibilities:
- Conduct comprehensive behavioral health screenings in a dynamic environment, including local county jails.
- Identify and maintain awareness of community resources for justice-involved populations and the enrollment process.
- Coordinate appropriate levels of care linkage to community behavioral health and ancillary services.
- Assess safety risks and provide crisis management interventions.
- Provide short-term clinical case management to ensure continuity of behavioral health and medical services.
- Review available behavioral health history to coordinate services and ensure appropriate levels of care for clients.
- Navigate and advocate within the behavioral health system of care on behalf of justice-involved clients as needed.
- Engage clients in the screening process and work collaboratively with a multi-disciplinary team to develop case plans, provide linkage to care based on individual needs, preferences, and objectives.
- Utilize motivational interviewing techniques to help clients identify and understand intrinsic goals and engage in behavior change.
- Participate in ongoing multi-disciplinary team meetings, providing education and consultation on linkage to behavioral health and ancillary services within the system of care.
- Provide clinical care consultations with California Department of Corrections and Rehabilitation personnel, probation officers, and community treatment providers.
- Maintain timely and appropriate clinical documentation records in San Diego County Probation and Behavioral Health Electronic Health records.
- Utilize technological systems to support clinical delivery of services, including access to SmartCare and Protected Health Information.
Requirements:
- Master's degree in Social Work, Marriage and Family Therapy, Psychology, Counseling, or a related field from an accredited university.
- Licensed eligible/registered associate with the BBS in Social Work, Psychology (Ph.D. or Psy.D.), Marriage and Family Therapy (AMFT), or Professional Clinical Counseling.
- Ability to provide licensure within six months of employment.
- Experience in direct clinical care in a behavioral health treatment setting, with an emphasis on assessment, case management, and crisis intervention.
