Job Summary: We are seeking a skilled Case Manager to join our team at Amigos de Guadalupe. The successful candidate will be responsible for providing comprehensive case management services to unhoused community members, including goal setting, long-term case plan development, and consistent support.

Key Responsibilities:

* Provide case management services to unhoused community members, including goal setting and long-term case plan development
* Champion money management guidance and tenant education to families
* Maintain a caseload of 50 families per year
* Meet with community members weekly or bi-weekly for 12 months and track their progress
* Connect community members with needed and available essential resources
* Follow-up with community members/agencies to document use/success of referrals

Requirements:

* BA in Social Work
* Minimum 2 years experience working in nonprofit setting
* Previous experience with case management of families
* Knowledge of community resources and counseling
* Excellent documentation, communication and written skills
* Ability to work in a variety of settings with culturally diverse individuals/families
* Ability to lift 25lbs
* Experience in working with the homeless population
