Job Description

We are seeking a skilled Nurse Practitioner Housecalls Statewide Traveler to join our team at UnitedHealth Group. As a member of our HouseCalls product, you will provide annual clinical assessments to patients in their homes, helping to improve their health and well-being.

Primary Responsibilities:

  • Conduct in-home assessments, including evaluations of past medical history and comprehensive physical exams
  • Perform evidence-based practice screenings and identify diagnoses for care management
  • Educate patients on disease processes, medications, and compliance

Requirements:

  • Active NP license or ability to obtain by start date
  • APCs must have approval to practice autonomously or without formal supervision from their licensing board
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
