Salary: $85,000 - $115,000 per year

Job Description

The successful candidate will have the opportunity to make a meaningful impact in the lives of our patients by providing high-quality, patient-centered care in the comfort of their own homes. This role requires excellent communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of a multidisciplinary team.

  • Provide comprehensive nursing assessments and develop individualized care plans in collaboration with physicians and other healthcare professionals.
  • Deliver evidence-based care that addresses the physical, emotional, and social needs of patients and their families.
  • Maintain accurate and timely documentation of patient interactions, including progress notes, medication lists, and treatment plans.
  • Collaborate with community resources and healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
Required Skills and Qualifications

To be considered for this role, you must possess:

  • A current RN licensure in the state of practice.
  • A valid CPR certification.
  • A reliable vehicle and valid driver's license (or access to public transportation).
  • Strong clinical judgment, problem-solving skills, and attention to detail.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
