Job Title: Home Health Care SpecialistWe are hiring a highly motivated Home Health Care Specialist to join our team at LHC Group. As a Home Health Care Specialist, you will work closely with patients and families to provide personalized care and support in the comfort of their own homes.
About the Job:- Develop and implement individualized care plans for patients in accordance with physician orders and applicable laws and regulations.
- Conduct ongoing assessments and evaluations to ensure patients' needs are met and adjust care plans as necessary.
- Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes.
- Ensure patients meet home health eligibility and medical necessity guidelines as defined by payer sources.
- Maintain accurate and complete records of patient interactions, including progress notes and treatment plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Estimated Salary: $80,000 - $105,000 per year
