Job Summary:
We are seeking a compassionate and skilled Registered Nurse to join our team as a Nurse Care Specialist. As a key member of our home healthcare team, you will provide direct patient care, educate patients and their families on disease management and prevention, and collaborate with other healthcare professionals to ensure comprehensive care.

Responsibilities:
* Provide direct patient care, including assessing patients' physical and emotional needs, developing and implementing individualized care plans, and evaluating patient outcomes.
* Educate patients and their families on disease management, medication use, and healthy lifestyle practices.
* Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
* Communicate effectively with patients, families, and caregivers to ensure seamless care coordination.

Requirements:
* Current RN licensure in the state of practice.
* Current CPR certification required.
* Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.

Benefits:
* Competitive salary range: $65,000 - $85,000 per year, depending on experience and location.
