Key Responsibilities:
- Conduct home visits and outreach activities to engage clients in the care process
- Develop and implement individualized care plans that address client needs and goals

Requirements and Qualifications:
- A bachelor's degree in a human services field
- 2+ years of community-based mental health experience as a case manager
