Care Support Professional

Reports to:

Program Director

Salary Range:

$45,000 - $60,000 per year, depending on experience.

We are seeking a highly motivated and compassionate Care Support Professional to join our team. As a key member of our staff, you will work closely with individuals to develop and implement personalized plans tailored to their unique needs and goals.

Responsibilities
  • Conduct initial assessments and develop individualized plans to promote independence and community integration
  • Provide daily living assistance and support to individuals with developmental disabilities or mental illness
  • Document progress and provide regular updates to the treatment team
  • Collaborate with other professionals to ensure seamless care coordination
Requirements
  • Bachelor's degree in a related field (e.g., psychology, social work, special education)
  • 1+ years of experience working with individuals with developmental disabilities or mental illness
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
