Position Summary:We are in search of a dedicated and empathetic Primary Care Provider to enhance our healthcare delivery. In this role, you will be instrumental in offering comprehensive medical services to patients residing in long-term care environments. This position presents a unique opportunity to positively impact the lives of patients while collaborating with a team of committed healthcare professionals.
Key Responsibilities:- Deliver comprehensive healthcare services and manage patient care effectively.
- Collaborate closely with a multidisciplinary team of healthcare experts.
- Evaluate, diagnose, and treat both the physical and mental health needs of residents.
- Conduct detailed patient evaluations, including reviews of medical histories and physical assessments.
- Order and analyze diagnostic tests to inform treatment decisions.
- Formulate precise diagnoses and develop tailored treatment plans.
- Prescribe medications and implement necessary treatments.
- Provide guidance and support to residents and their families.
- Work in conjunction with the interdisciplinary team to ensure seamless care coordination.
- Oversee chronic condition management and track patient progress.
- Adjust care plans as needed to meet evolving patient needs.
- Commit to delivering person-centered care.
- Establish a safe and supportive atmosphere for elderly residents within the facility.
- Active Nurse Practitioner or Physician Assistant License.
- Current DEA License.
- Minimum of 6 months experience in long-term care settings.
- Paid Time Off.
- Malpractice insurance coverage.
Salary Range: $90,000 - $120,000.00
