Job Title: Case Manager

Join The People Concern team as a Case Manager and make a meaningful impact in the lives of vulnerable individuals. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive services to support clients in regaining control over their lives.

Job Summary:

The Case Manager will be responsible for identifying, engaging, and providing support to homeless clients, including referrals, linkages, and coordinating care. This role requires a strong understanding of issues faced by homeless individuals, including mental health, substance addiction, and physical health problems.

Key Responsibilities:
  • Utilize harm reduction, social, environmental, and motivational strategies to support clients in collaboration with clinicians and other team members.
  • Encourage and promote a strength-based environment to assist clients in meeting their individual goals.
  • Conduct street outreach to engage homeless individuals in services and treatment.
  • Assist clients in acquiring permanent housing using various resources.
  • Provide crisis intervention, basic services, and short-term and long-term case management services.
  • Develop effective, trusting relationships with clients to facilitate their move towards independence.
  • Complete all charting in a timely manner.
  • Provide information, referrals, and social services advocacy.
  • Network with agency and outside service providers to ensure client access to appropriate resources.
  • Provide life skills counseling as appropriate.
  • Collaborate and communicate effectively with police officers and local hospital staff to reduce police contact and ED visits.
  • Work effectively on an interdisciplinary team.
Requirements:
  • Bachelor's degree or equivalent experience.
  • Minimum of two years in case management.
  • Demonstrated knowledge of issues faced by homeless individuals.
  • Skilled in non-violent crisis intervention.
  • Ability to self-motivate, work independently, and communicate effectively.
  • Able to work collaboratively as part of an interdisciplinary team.
  • Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
  • Computer literate, able to use computerized database system for client file management.
Preferred Qualifications:
  • Spanish-speaking.
Work Environment:

Mostly field work with shared office space. No remote work option.
