Key Responsibilities- Manage a caseload of clients with moderate to complex medical and psychosocial needs, providing comprehensive care coordination services to ensure optimal health outcomes.
- Develop, implement, and update individualized care plans in collaboration with healthcare providers and other multidisciplinary team members.
- Conduct regular assessments and evaluations to identify needs and barriers, and close gaps in care.
- Collaborate with Senior Care Managers to review and prioritize cases, set objectives, and identify potential risk and utilization concerns.
- Communicate effectively with clients, families, healthcare providers, and other stakeholders to ensure seamless care coordination and transitions.
- Document client interactions and care plans accurately and timely, adhering to organizational policies and national standards.
- Participate in multidisciplinary meetings and contribute to the development of care plans and strategies.
- Stay up-to-date with changing strategies and trends in care coordination and care management.
- Master's degree in Social Work or related field.
- Two or more years of experience in healthcare or related field, with a focus on care management and coordination.
- Licensed in DC as a Licensed Independent Social Worker (LICSW).
- Effective verbal and written communication skills, with the ability to communicate complex information in a clear and concise manner.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
- Ability to work in a fast-paced environment and adapt to changing priorities and deadlines.
