Patient Care Navigator

Estimated Salary: $60,000 - $80,000 per year.
The Opportunity: We're seeking a skilled Registered Nurse to join our team as a Patient Care Navigator. In this role, you'll have the opportunity to provide comprehensive nursing care to patients in their homes, working collaboratively with other healthcare professionals to achieve continuity of patient care.
Your Responsibilities:
- Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop personalized plans.
- Coordinate total plan of care with other healthcare professionals, including physicians, agency personnel, patients, and family members.
- Ensure patients meet home health eligibility and medical necessity guidelines, as defined by payer sources.
- Maintain accurate, complete, and timely documentation, collaborating with physician orders.
- Perform patient comprehensive assessments at designated time points and develop appropriate plans of care.
Requirements:
- Current RN licensure in the state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
