Job Description

We are seeking an experienced Patient Care Coordinator to join our team at Sanzie HealthCare Services Inc. In this role, you will be responsible for coordinating patient scheduling, tracking supervisory visits, and ensuring consistency in patient schedules.

The ideal candidate will possess strong communication skills, be able to multitask efficiently, and have a solid understanding of healthcare operations. They should also have a high level of attention to detail and be able to work effectively in a fast-paced environment.

Key Responsibilities:

  1. Manage patient scheduling and tracking of supervisory visits.
  2. Communicate with patients, families, and caregivers to ensure seamless care delivery.
  3. Coordinate administrative tasks, including data entry and documentation.
  4. Ensure compliance with regulatory requirements and agency policies.

Requirements:

  • Associate degree or equivalent.
  • 2 years of Customer Service experience required.
  • 2 years of Healthcare or home care experience preferred.

Salary and Benefits:

  • A competitive salary range of $55,000 - $75,000 per year.
