Patient Care Navigator

We are seeking a highly skilled Patient Care Navigator to join our team at Cityblock. In this role, you will be responsible for managing a panel of patients with rising and high intensity needs to support integrated chronic disease and behavioral health care.
About the RoleYou will collaborate with patients to create a care plan and oversee progress to the plan, frequently reassessing needs, coordinating with providers and specialized resources, and partnering closely with the Community Health Partner to build trust and demonstrate advocacy.
Responsibilities- Engagement: Receives members from engagement and care team, describes program expectations and goals to members
- Assess/Intake: Completes self-efficacy and relevant condition-specific screeners with members, triages members with need for behavioral health programming, conducts in-person clinical exam if appropriate
- Case Review and Care Planning: Prepares and participates in case conference, develops care plan with the member, collaborates with Community Health Partner on social needs as needed
- Clinical Visits and Follow-Up: Regular check-ins with member as guided by program, follows up on routine therapeutic interventions, addresses and responds to member needs and delegates tasks in timely fashion
- Graduate of an accredited school of nursing (R.N.)
- Basic Life Support (BLS) certification is a requirement
- Active RN License in the state(s) practicing
- Unrestricted Driver's License
- 3+ Years of Experience
- Comfortable using technology to support members without in-person contact
- Understanding of how to use scheduling platforms to ensure accurate appointment scheduling and management
- Proficient in collecting member clinical and demographic data and documenting appropriately in a timely manner
The estimated salary for this role is around $80,000 per year, plus benefits including medical, dental, and vision insurance, 401(k) matching, and paid time off.
