Key Responsibilities:
- Develop and implement individualized care plans for patients, ensuring their needs are met and exceeded.
- Coordinate with home health aides to ensure timely and effective delivery of care services.
- Manage last-minute call-outs and find suitable coverage to minimize disruptions to patient care.
- Develop and maintain an on-call pool of home health aides with various shifts in coordination with Human Resources.
- Communicate effectively with referral sources, case managers, and patients to provide exceptional customer service.
- Match patients with the most suitable caregivers based on their needs and preferences.
- Manage a caseload of 100+ patients, ensuring timely and effective care delivery.
- Review care plans with patients and caregivers to ensure alignment and satisfaction.
- Resolve caregiver and patient grievances and complaints in a timely and professional manner.
- Monitor and address overutilization of hours to ensure efficient care delivery.
- Report personnel performance issues to management.
- Contribute to team efforts by accomplishing related results as needed.
Requirements:
- High school graduate or equivalent.
- 1-2+ years of experience in a home care or service coordination role.
- Demonstrated capability to maintain strict confidentiality.
- Proven ability to generate leads and monitor referrals.
- Strong typing and computer skills.
- Comfortable with closing and asking for business.
- Well-organized, accurate, and attentive to detail.
- Excellent communication, public relations, and follow-up skills.
- Experience with HHA Exchange and EVV.
- Bilingual English/Spanish preferred.
