RN Care Coordinator

Highmark Health seeks a skilled RN Care Coordinator to join our team. As a Care Coordination RN, you will play a critical role in ensuring seamless transitions of care for our patients. Your expertise in utilization review and care management will be instrumental in promoting optimal resource utilization and patient satisfaction.
Key Responsibilities:
- Assess patient physical, psychosocial, and economic needs for effective transition of care planning
- Collaborate with the care team to develop and communicate the continuum of care transition plan
- Document, verify, and validate specific data required to monitor and evaluate interventions and outcomes
- Interview and collect patient specified data and chart review related to readmission
Requirements:
- 2 years of recent clinical RN Acute care, Home care, Palliative, Hospice, or Care Management experience
- RN license in appropriate state based on location
- CPR Certification
Preferred Qualifications:
- Discharge planning and community resource knowledge
- Care facilitation and utilization review experience
Work Environment:
This is an office-based position that requires occasional travel to various work sites or from site-to-site. The ideal candidate will have excellent organizational and time management skills, as well as the ability to work in a high-performing team environment that requires flexibility.
Pay Range: $57,700.00 - $106,700.00
