We are seeking a skilled Clinical Admission Nurse to join our team at Hospice of the Western Reserve. As a key member of our interdisciplinary team, you will be responsible for assessing patient needs, providing information and education, and promoting our hospice philosophy of care.

About the Position

This role requires strong communication and interpersonal skills, as well as the ability to work collaboratively with patients, families, and healthcare professionals. Key responsibilities include providing nursing care, emotional support, and education to patients and families, utilizing computer skills to access medical information and documents, and promoting teamwork by participating actively and appropriately as a member of an interdisciplinary team.

Key Responsibilities:
  • Performs all duties and responsibilities in accordance with the Nurse Practice Act and basic principles of professional nursing
  • Provides information assessment, and admission services to patients who have been referred to HWR
  • Communicates with Referral Department, referring physicians, and hospice medical directors to ensure appropriateness of referrals and obtain necessary certifications
  • Maintains knowledge and skills for hospice care in all settings, including regulatory and reimbursement guidelines
Requirements
  • Graduate of an accredited nursing program: BSN and/or CHPN experience preferred
  • Required Nurse Licensure by the state of Ohio, valid driver's license with an automobile insured in accordance with state and agency requirements
  • Minimum of one year of nursing experience required, including intake, home care, hospice, and/or oncology nursing experience
Salary and Benefits

We offer a competitive salary range of $65,000-$85,000 annually, depending on experience, plus a comprehensive benefits package, including health, dental, vision, retirement, and tuition assistance. This is an exempt role with a technology package and protocols in place for well-being during COVID-19.
