Job Summary

Hospice of the Western Reserve seeks an experienced Clinical Admission Nurse to join our team in delivering exceptional patient care and coordination. As a key member of our interdisciplinary team, you will work closely with patients, families, and referral sources to ensure seamless care transitions.

About the Role:

  • You will perform assessment visits, communicate with referral sources, and provide information, assessment, and admission services to patients and families.
  • You will apply nursing principles and techniques to deliver exceptional care, promote teamwork, and maintain accurate records.
  • You will stay updated on regulatory and reimbursement guidelines, collaborate with IDT members, and contribute to efficient and financially responsible outcomes.

Key Responsibilities:

  • Perform assessment visits to ensure appropriateness of referrals and obtain necessary certifications.
  • Communicate with the Referral Department, referring physicians, and hospice medical directors to coordinate admissions.
  • Provide information, assessment, and admission services to patients and families, applying nursing principles and techniques.
  • Maintain accurate and up-to-date records, utilizing computer skills to access medical information and documents.

Requirements:

  • Graduate of an accredited nursing program; BSN and/or CHPN experience preferred.
  • Valid Ohio nurse licensure, valid driver's license, and automobile insurance meeting state requirements.
  • Minimum one year of nursing experience required; intake, home care, hospice, and/or oncology experience desirable.

Estimated Salary: $73,000 - $90,000 per annum, based on experience and qualifications.
