About the Role

The Clinical Admission Nurse is responsible for assuring that HWR programs are effectively represented to patients, families, and others who have an interest in hospice services. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care.

Main Responsibilities:

  1. Performs duties and responsibilities in accordance with the Nurse Practice Act and professional nursing principles.
  2. Provides information assessment, and admission services to patients referred to HWR.
  3. Communicates with the Referral Department, referring physicians, and Hospice MDs to ensure appropriateness of referrals and obtain necessary certifications.
  4. Keeps the Referral Department informed about the progress of referrals/admissions.
  5. Applies nursing principles and techniques in patient treatment, providing emotional support and education to patients and families.
  6. Promotes teamwork by actively participating as a member of an interdisciplinary team, sharing essential patient information to coordinate care.
  7. Completes and submits documentation within agency timelines, reflecting complete, legible, and accurate care given.
  8. Maintains knowledge and skills for hospice care in all settings, including regulatory/reimbursement guidelines.

Requirements:

  • A graduate of an accredited nursing program (BSN and/or CHPN).
  • Required Nurse Licensure by the state of Ohio and a valid driver's license with automobile insurance.
  • Minimum one year of nursing experience, preferably in intake, home care, hospice, and/or oncology nursing.
