Job Description">

Hospice of the Western Reserve seeks a skilled and compassionate Licensed Practical Nurse to join our team. As a LPN, you will work with patients, families, and healthcare professionals to deliver individualized care that addresses the physical, emotional, and spiritual needs of those we serve.

">Key Responsibilities">
  • Deliver high-quality patient care in accordance with HWR clinical standards, policies, and procedures
  • Develop and maintain accurate and detailed records of patient care
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care
  • Participate in care conferences, bereavement follow-up, and quality improvement activities
Requirements and Qualifications">
  • Licensed LPN in the State of Ohio
  • Experience in hospice care, home health, oncology, or pain and symptom management preferred
  • CPR certification required
  • Valid driver's license, good driving record, and auto insurance coverage

We offer a competitive salary of $62,000 - $72,000 per year, depending on experience, along with a comprehensive benefits package, including tuition assistance, a technology package, and protocols for maintaining your wellbeing during COVID-19.
