Care Management Specialist

The Registered Nurse in Care Management will work within a collaborative healthcare team to assess, plan, implement, and evaluate the services needed to meet the health needs of patients. Key Responsibilities include:
- Utilize a collaborative process to assess, plan, implement, and monitor services needed for patient care.
- Manage the patient's length of stay and ensure efficient use of inpatient resources.
- Conduct assessments using standardized criteria to determine medical necessity and appropriateness of admission.
- Support the coordination of care across multidisciplinary teams.
- Evaluate patients' needs to determine what interventions, resources, and services are necessary to improve outcomes.
- Ensure compliance with payer contracts.
- Communicate effectively with patients, families, and interdisciplinary teams to manage care, provide education, and ensure smooth transitions throughout the patient's hospital stay.
- Maintain accurate, up-to-date documentation in the electronic health record (EHR) system.
- Advocate for patients' care needs and collaborate with physicians and healthcare providers to ensure that care plans align with clinical guidelines and patient preferences.
