Ambulatory Care Nurse Specialist

We are seeking an experienced Ambulatory Care Nurse Specialist to join our esteemed HIV primary care team. As a key member of our team, you will play a critical role in promoting harm reduction, linkage, and retention to HIV primary care, empowering individuals experiencing homelessness and living with HIV to achieve optimal health outcomes.
Key Responsibilities:
- Deliver high-quality outpatient primary care nursing services, including medical assessments and triage for HIV team patients.
- Collaborate with integrated HIV team behavioral health staff to ensure seamless care coordination.
- Conduct home visits and outreach activities to provide tailored nursing care and support to patients.
Required Qualifications:
- Licensed as a Registered Nurse in Massachusetts.
- Bachelor's degree in nursing preferred; 2 years of experience in ambulatory care required.
- Certification as AIDS Certified Registered Nurse (ACRN) is an asset.
Compensation and Benefits:
