Position Overview
This role is responsible for overseeing the medication management of designated pediatric patients. The pharmacist will assess and monitor medication therapy for suitability based on the patient's health condition, institutional guidelines, and cost-effective practices. Key responsibilities include medication reconciliation, monitoring, and providing patient education.

Educational Responsibilities
The position involves active participation in the education of Pharmacy and Medical Residents, along with providing necessary training to healthcare professionals. The pharmacist will also contribute expert pharmacy knowledge for various documents such as order sets and formularies.

Qualifications
To excel in this role, candidates must meet the following requirements:

  • Education: A Bachelor of Science in Pharmacy is required. Completion of a PGY1 Pharmacy Practice Residency is preferred, or three years of relevant specialty experience may be accepted in lieu of the residency. A Doctor of Pharmacy (PharmD) and completion of a PGY2 Specialty Pharmacy Residency are also preferred.
  • Experience: No prior work experience is required.
  • Licensure: A current pharmacy license in the state of practice is mandatory.
  • Skills: Proficiency in computer applications and data entry is essential. Strong verbal and written communication skills are necessary to convey information clearly to various stakeholders.

Core Responsibilities
The pharmacist will provide pharmacokinetic consultations, utilizing appropriate laboratory data to evaluate drug selection and dosing, and communicate findings to healthcare practitioners. Evaluating patient responses to drug therapy is crucial, with a focus on documenting interventions and conducting outcome studies to assess clinical activities.

The role includes assisting in the development of clinical pathways, best practices, and guidelines tailored to the pediatric population. Additionally, the pharmacist will educate staff, students, patients, and other healthcare professionals, while also mentoring residents and new staff to achieve departmental objectives.

Patient Care Commitment
Beyond hospital visits, the pharmacist will ensure comprehensive patient care throughout all transitions. Flexibility in accommodating the organization's needs during staffing shortages is expected.

Compliance and Standards

Physical Demands
