Job Summary:

We are seeking a skilled Nurse Practitioner to provide high-quality medical care to seniors and patients with complex needs.

About the Role:

This is an exciting opportunity for a compassionate and experienced Nurse Practitioner to join our team at Abode Care Partners. As a key member of our interdisciplinary team, you will be responsible for delivering evidence-based medical care to our patients in various settings.

Key Responsibilities:

  • Perform comprehensive assessments of patients' medical needs and develop individualized care plans
  • Provide routine and episodic medical care, including treating chronic and urgent illnesses, minor injuries, and other health issues
  • Order and interpret laboratory and diagnostic testing as necessary
  • Collaborate with healthcare professionals to ensure seamless patient care and maintain accurate records
  • Make referrals to other healthcare practitioners when needed

Requirements:

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
  • Proficiency in medical instruments and equipment
  • Knowledge base of HIPAA and OSHA regulations
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications

Salary and Benefits:

The estimated salary for this role is $110,000 - $130,000 per year, depending on experience and qualifications.
