Job Overview

We are seeking a skilled Geriatric Care Team Coordinator to join our team at The Residence Paine Estate in Wayland, MA. As a key member of our Resident Care Department, you will play a critical role in ensuring the highest level of quality care for our residents.

Responsibilities
  • Assist the Resident Care Director with the management of the community, including direct supervision of Resident Care Associates and monitoring emerging health needs of residents.
  • Cover for resident care staffing as needed and assist with on-call duties.
  • Follow up on family issues as directed by the Resident Care Director.
Essential Job Functions
  1. Promote the mission and core values of LCB Senior Living, LLC to residents, visitors, coworkers, and the surrounding community.
  2. Ambassadorial role in growing community occupancy by representing and interacting positively with potential residents, family members, and professionals.
  3. Assist with the evaluation of resident needs and development of service plans according to state regulatory requirements.
  4. Demonstrate understanding of pathophysiological and psychosocial changes associated with aging and disabilities.
  5. Direct and supervise daily work assignments of resident care associates.
  6. Evaluate resident issues and medical concerns, discussing with the ED, family, and MD as necessary.
  7. Encourage teamwork through cooperative interactions with all departments to eliminate/correct problem areas and improve services.
  8. Monitor resident care assistants' compliance with infection control/sharps disposal program.
  9. Participate in community marketing efforts.
  10. Provide support to resident care associates regarding assignment questions or concerns, as well as emergency situations.
  11. Ensure required staffing and schedules are met at all times, including backup coverage for staff.
  12. Complete and review incident/accident reports, implementing changes through proper channels to minimize future issues.
  13. Assist the Resident Care Director with documentation required by LCB Senior Living, LLC or state regulations.
  14. Administer topical and oral medications, ear and eye drops, and injectables with written physician orders.
Qualifications

To be successful in this role, you should possess:

  • A high school diploma and graduation from an accredited Nursing Program.
  • At least two years of experience in a geriatric setting and previous healthcare experience in hospital and/or community settings.
  • Current RN or LPN License in the state of employment.
