The Role

We are seeking experienced Licensed Practical Nurses (LPNs) to join our team as Home Health Nurse Specialists in West Michigan. In this role, you will provide home-based care to patients with illnesses, injuries, and chronic diseases, often after hospital discharge, under the supervision of an RN.

Salary and Benefits

As a Home Health Nurse Specialist with Interim HealthCare, you can expect a competitive salary range of $55k - $65k annually, with weekly bonus opportunities. Responsibilities

  • Provide high-quality patient care, including taking vitals, doing assessments, documenting progress, and reporting changes to the RN supervisor.
  • Collaborate with a multidisciplinary team to coordinate patient care and develop plans for discharge.
  • Educate family members on patient care, medication, and home safety.
  • Work closely with RNs, CNAs, and aides to deliver exceptional patient outcomes.

Requirements

  • Licensed Practical Nurse (LPN) with active, unrestricted license in the State of Michigan.
  • At least one year of clinical experience as a Licensed Practical Nurse (LPN).
  • Current CPR Certification.
  • Reliable transportation with a valid state driver's license and current car insurance.
