Salary and Benefits

The estimated salary range for this position is $56.95 - $78.30 per hour, with the potential for a $9,000 sign-on bonus (conditions apply). Our competitive benefits package includes comprehensive health insurance, paid time off, and opportunities for professional growth and development.

Job Description

Hospice Nurse Role

  • Admit patients to service and assess their physical, psychosocial, environmental, safety, and developmental needs.
  • Develop an individualized plan of care based on assessment and document observations and interventions related to the patient's environment.
  • Coordinate care with a multidisciplinary team, participate in quality assessment performance improvement programs, and conduct follow-up visits as needed.
Responsibilities and Qualifications

Key Responsibilities:

  • Perform initial comprehensive assessments.
  • Document observations, clinical findings, problems, skilled interventions, goals, and discharge plans.
  • In consultation with the assigned Clinical Team Manager, initiate and regularly re-evaluate and revise plan of care.

Qualifications:

  • Current California RN license.
  • Minimum of one year of nursing experience required.
  • Current CPR Certificate.
  • Current California driver's license and automobile insurance.
