We are seeking a highly skilled Clinical Dietitian to join our team at Penn Medicine Lancaster General Health. As a Clinical Dietitian, you will play a critical role in providing medical nutrition therapy and nutrition education to patients of all ages and to members of the community.
Key Responsibilities- Participate as a member of the LGH healthcare team and provide medical nutrition therapy and nutrition education to patients and community members.
- Serve as a resource for nutrition information to various allied health personnel and assist in integrating the patient's nutritional care into the total treatment plan.
- Identify patients at nutritional risk, categorize level of malnutrition based on ASPEN/AND criteria if present, initiate nutrition intervention(s) as indicated and assign time frame for follow-up care according to established standards.
- Develop and implement nutritional care plans, communicate plans to healthcare team by verbal and written documentation, assess outcomes of nutrition interventions, and revise nutrition care plans to attain goals and desired outcomes.
- Maintain accurate electronic record of care plan, use electronic care plan as an effective communication tool with fellow nutrition staff, and analyze physiologic and metabolic responses to PENTailor nutrition support regimens to the needs of the individual per clinical data and outcomes.
- Evaluate social, economic, and demographic factors prior to discharge planning for patients receiving PENDemonstrate knowledge of Medicare guidelines regarding PENObtain required continuing education to maintain professional growth, development, and competency.
- Calculate nutrient requirements and recommend PEN formulations to physicians, attend multidisciplinary team rounding as appropriate and necessary to promote nutrition outcomes as required by patient unit assignment, educate individuals and families in nutritional principles, therapeutic dietary plans, food selection, food economics, adapt nutrition plan to the individual's lifestyle, teach group nutrition classes, and interpret, evaluate, and apply current research and evidence-based information related to nutritional care.
- Function as a liaison to the Food Service Department to promote patient satisfaction, assist with patient meal planning when needed, and provide clinical guidance to dietetic technicians.
- Participate in designing and conducting performance improvement initiatives with physicians, nurses, and ancillary departments (such as radiology, pharmacy, speech).
- Bachelor of Science degree in nutrition or related field from an accredited school.
- Certification as a Registered Dietitian, issued by the Academy of Nutrition and Dietetics, or registration eligible/obtaining certification within 3 months from date of hire.
- Licensed in the state of Pennsylvania as a dietitian-nutritionist within three (3) months from date of hire.
- One (1) to two (2) years of clinical nutrition experience and education.
- One (1) year education background in teaching/learning with excellent interpersonal skills.
- Two (2) to three (3) years of clinical nutrition experience and education skills.
- One year of Parenteral and Enteral Nutrition support experience.
