Position Description:
The Registered Dietitian (RD) will be responsible for providing nutrition counseling and support to patients of JWCH, Wesley Health Centers. The RD will develop and implement personalized nutrition care plans, conduct nutritional assessments, and educate patients on healthy eating practices to promote overall health and well-being. This role involves collaborating with healthcare providers to integrate nutrition into patient care, contributing to JWCH's mission to improve the health status and well-being of underserved populations of the Los Angeles Area.
Principal Responsibilities:
- Conduct comprehensive nutritional assessments for patients, including dietary histories and lifestyle evaluations.
- Participates in the development of nutrition program by identifying the nutrition needs of individuals at risk, defining the problems, which may be prevented or corrected with proper nutrition
- Monitor patient progress and adjust nutrition plans as needed to ensure optimal outcomes.
- Provide one-on-one nutrition counseling and education to patients on topics such as weight management, chronic disease management, and general wellness.
- Conduct group nutrition education sessions and workshops for patients and community members.
- Work closely with physicians, nurses, and other healthcare professionals to integrate nutrition care into overall patient treatment plans.
- Communicate effectively with patients and their families to provide support and guidance on nutrition-related issues.
- Develop and implement nutrition programs and initiatives to address community health needs.
- Evaluate the effectiveness of nutrition programs and services, making improvements as necessary.
- Maintain accurate and up-to-date patient records, including progress notes and care plans.
- Ensure compliance with clinic policies, procedures, and regulatory requirements related to nutrition and dietetics.
- Stay current with the latest research, trends, and best practices in nutrition and dietetics.
- Participates as member of CQI and other assigned committees.
- Performs other tasks as assigned.
- Bachelor's degree in Dietetics, Nutrition, or a related field from an accredited institution.
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) Certification required.
- At least two years of experience in a related field of dietary work.
- Experience working with diverse patient populations and managing chronic conditions.
- Strong knowledge of nutrition science, dietary guidelines, and evidence-based practices.
- Excellent interpersonal and communication skills, with the ability to educate and motivate patients effectively.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in using electronic health records (EHR) and other relevant software.
- Strong organizational skills and attention to detail.
- Ability to handle sensitive information with confidentiality and professionalism.
- Current California Driver's License.
- Ability to travel to multiple work sites.
- Bilingual (English/Spanish) ability.
