We are seeking a highly skilled LTSS Service Coordinator - Clinician to join our team at Elevance Health. As a key member of our care coordination team, you will play a critical role in ensuring the delivery of high-quality, patient-centered care to our members.
Key Responsibilities- Assist in identifying members who require high-risk interventions and develop strategies to mitigate potential complications.
- Collect and analyze clinical data to inform care decisions and identify opportunities for cost-effective and efficient care.
- Collaborate with healthcare teams to develop and implement comprehensive care plans that meet the unique needs of each member.
- Participate in care coordination activities, including telephonic and face-to-face assessments, to ensure seamless transitions of care.
- Utilize clinical judgment and decision-making skills to identify opportunities for alternative levels of care or other waiver programs.
- LPN/LVN, LSW, LCSW, or LMSW license in accordance with applicable state law and a minimum of 2 years of experience in a Service Coordinator or similar role.
- Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
- Experience using Microsoft Office Suite, including Outlook, Word, and Excel.
- Previous case management experience.
- Ability to travel to worksite and other locations as necessary.
